Are you currently lactating (or have recently ceased) and are suffering from all or any of these symptoms: pain, redness, tightness & tenderness in your breasts? Feeling generally off? Know something doesn't feel right? Mastitis isn't something that should be left to "watch and wait" as it can worsen very quickly & make you quite unwell. For an out of hours, emergency appointment, please fill out our Mastitis Booking Form & I will get back to you ASAP & hopefully we can make something work. I will always bring my ultrasound machine to these consults & provide you with advice and treatment to hopefully catch it before it worsens. Please don't wait to get help, and always chat to your lactation consultant, GP or medical provider.

Cancellation Policy, Terms & Conditions, Informed Choice and Payer Information

By booking an appointment at Port Lincoln Women's Physio, you are agreeing to the following Terms & Conditions:

Cancellation Fees

I know life, especially with little people around, can be very unexpected! For this reason, I try to be as flexible as possible, however, a minimum of 4 hours notice is required to cancel or reschedule your upcoming appointment. If you fail to provide the required notice, you will be charged a 50% fee of the consult type you were booked in for (see above). For example, if you were booked in for a Women’s Health Initial Assessment, you would be charged a $100 cancellation fee. If you were booked in for a review appointment, you would be charged $65. This fee is not claimable on your private health insurance, and you would have to cover the cost independently. If you cancel/change your appointment and have provided more than 4 hours notice, you will not incur any fees. Should you have paid a 50% deposit for your appointment already, this amount will be forfeited as your cancellation fee. If you had not been required to pay a deposit for your session, you will be emailed, or posted, an invoice that requires payment within 14 days.

Missed Appointment Fees

If you simply do not attend your appointment, or are not at your nominated address at the time of your scheduled appointment, or if you turn me away, without having attempted to cancel earlier via text, email or phone call/voicemail, you will be charged the 100% fee for the consult type you had booked (see above).  Again, this is not claimable on your private health insurance as they will only provide a rebate on services you attended. I will endeavour to make reasonable contact with you, however if I have not heard anything from you after 15 minutes, your session will be forfeited as a "Did Not Attend" fee, of 100% of the above appointment cost. You will be emailed, or posted, an invoice which requires payment within 14 days.

How to pay for your appointment

I have a mobile HICAPS terminal, where we can swipe your private health card, and you'll just have to pay the gap on the spot. **Please note, not all Health Funds allow us to use a mobile terminal unfortunately. Click this link to see if your fund will**. If your health fund is not on this list, you will need to pay in full either via cash or card, and I will be able to email or post you a copy of the paid invoice for you to claim back manually with your health fund afterwards. If you want to know in advance what your gap will be, you will need to call your health fund directly and quote the item code(s) and our fees that you will find on the Fees page.

If you do not have private health insurance for Physiotherapy, you are required to cover the full price of the consult independently via either card (preferred) or cash. Please note, I do not accept payment via cheques. If you wish to pay in cash, please endeavour to have the correct amount where possible.

All fees must be paid in full on the day of service.

Third Party Payers

Unfortunately at this time, I am not able to take on any clients wishing to pay via any third party, for example, Return to Work SA, DVA, MVA, NDIS, My Aged Care, and the like. This also includes Medicare EPC/TCA scheme clients.

Informed Choice

It is important to us that you always feel comfortable and in control of what is happening with you and your body. You always have the right to refuse any treatment or assessment. Where possible, your Physiotherapist will explain any risks associated with any part of your session, as well as the benefits, alternative options, and anything else to note if you did choose not to go ahead with a treatment or assessment. You may withdraw your consent at any stage. Consent may be implied, given verbally, or written. Any client receiving our service that is legally unable to provide their own consent must be accompanied by someone who is legally able to make decisions on their behalf for the duration of all sessions.

Terms & Conditions

By booking an appointment with Port Lincoln Women's Physio, you are agreeing to the above Cancellation Policy & Terms & Conditions. You acknowledge the above Informed Choice information and provide your consent for assessment and treatment/management strategies. You acknowledge that these policies and terms may be changed without notice.
